To ensure that children who require educational support receive the appropriate care, referrals to River Tees Hospital School should be made by the child home schools. It is essential to follow our admissions policy, which outlines the referral process, and the necessary documentation required. Referrals should always include relevant medical information and details about the child’s educational needs to facilitate a smooth transition into our school. 

By collaborating closely with the child’s home school and medical team, we can create a tailored educational plan that supports the child’s learning and wellbeing during their recovery period.

Referral FAQs

A referral is a way to inform us about a child or young person who is struggling to attend their usual school due to physical or mental health issues. Referrals must come from the child’s home school, with supporting medical documentation.

You can make a referral by completing the relevant referral form depending on which Local Authority a child is from.

To ensure a smooth referral process, please include the following information:

  • Consent from the young person and their parents/carers
  • Consent from the Head Teacher of the child’s home school
  • Comprehensive background information, including any available reports and assessments
  • Detailed medical evidence from a healthcare professional, who is working directly with the child.
  • Details on how the school has implemented a graduated response which addresses severe difficulties in attending school due to emotional factors.

Please note: Any mandatory fields on the referral that are incomplete will be declined at the first panel and referred to the home school for completion.

The Inclusion team at the relevant Local Authority will verify that all information is accurate before presenting it to the multi-agency panel. The Inclusion Officers may reach out for further clarification if needed.

The duration of the referral process can vary based on when the form is submitted. The panel meets every 4 weeks to review submitted referrals.

If the referral is not successful, the Inclusion Officers will notify the individual who completed the form, explaining the outcome, the reasons behind it and suggested actions to follow.

All referrals must be supported by a medical professional to be considered for a place at Hospital School.
