Reintegration & Transition

At River Tees Hospital School, the needs of each pupil are at the heart of our support as they prepare to reintegrate into a mainstream setting or transition to a special school. We prioritise clear communication with all parties involved, including the pupil and their family, to ensure a smooth transition.

Each pupil will have a bespoke reintegration plan developed collaboratively with the parent or carer, the pupil, the referring school, and the professionals working with the family. This personalised approach is designed to address the unique needs of everyone.

We conduct regular reviews of the reintegration plan every six weeks to enhance the pupil’s opportunities for successful integration and to identify and address any barriers that may hinder their progress.

Our aim is for every pupil transitioning to feel supported throughout their journey and to achieve success.

Following a placement at River Tees Hospital School, our staff collaborate closely with the transitioning school to agree on a comprehensive reintegration plan. This plan may include:

The school and pupil will receive assistance from trained staff from the outset of the placement to ensure a clear understanding of the pupil’s needs.

Together, we will establish clear goals and expectations regarding the duration of support, and how the plan will be monitored. This information will be communicated effectively to all involved, including the pupil and their family.

The key worker will identify any necessary adjustments that the school should implement to facilitate a successful reintegration.

The pupil will have access to specialised support from River Tees Hospital School staff tailored to their specific needs prior to their return and during the initial stages of reintegration. This may include personal development interventions, reading support, or mental health assistance.

Each pupil will receive bespoke support from a key worker, who will provide advice and guidance to the pupil, their family, and the transitioning school.

We are committed to offering continuous support to the pupil and their family to enhance the likelihood of a settled and successful reintegration or transition.
